Simple:

1. Update your will.
2. Write letters to loved ones before departure, give them to the kid next door and tell him to go on standby.
3. Get finances in order.
4. Plan and pay for funeral service. Costco has discounts on caskets, you can prepay online.
5. Ensure your insurance is topped up and add the webmaster of this forum as a beneficiary, money to be used for future generations to pay those that keep the pools and surrounding areas clean. The authorities may even be convinced to have a plaque installed with your name.
6. Write a mail us a blank check payable to SB Sheriff Search and Rescue. We will fill in the amount for the helicopter lift. May want to send us two if they bill for fuel separately.
7. Drive to DCHS and hit the trail.

Seriously, if you have two weeks your heart condition is more important than how much you weigh. Prepare for an uphill climb with a 900 ft elevation change on way out. Winter is the best time to go on the coolest of days. Bring lots of water and a salty snack. Stop a lot. Stretch. You know your body best not your doctor. A disclaimer: I'm not a physician (yet) ... although I have worked with them side by side for years and after a few have challenged me to an IQ test which we took together and med school entrance tests ... and after creaming every single one of their butts on the first, coming in within 3 points of their best doc and breezing through the latter, I may get in on the action so they start taking me more seriously and I can give out advice like this and charge for it, just kidding.

A few tips for the next 2 weeks:

1. When shopping, pick the farthest parking spot from the store, then walk. You may save your car from a potential dent as well. I do this everywhere I go.
2. If under 3 floors, take the stairs, not the elevator.
3. If store is within a mile or two, take a bike instead of the car.
4. Reduce sugar intake, increase fruits and veggies.
5. Get plenty of sleep.

You have many choices with your Subaru. You can browse a few historical posts for suggestions.
